The American Cancer Society (ACS) has developed evidence- and consensus-based cancer survivorship care guidelines to help primary care and other clinicians provide high-quality, comprehensive, coordinated clinical follow-up care to adult post-treatment cancer survivors to promote optimal health and quality of life.

Cancer survivors need coordinated monitoring for cancer recurrence, screening and early detection of second primary cancers, assessment and management of potential physical and psychosocial effects of cancer and its treatment, counseling on health promotion strategies for nutrition, physical activity, tobacco cessation, and alcohol consumption, as well as coordination of care utilizing survivorship care plans, communication with other providers, and inclusion of caregivers.

Download the official ACS app for providers of clinical follow-up care to adult post-treatment cancer survivors. Access content adapted from the ACS cancer survivorship care guidelines from your iPhone at the point-of-care. Share the app with your network of colleagues, especially those in primary care.

These guidelines address the cancer survivorship care needs of breast, colorectal, head and neck, and prostate cancer survivors. ACS guidelines for nutrition and physical activity for cancer survivors are also included.
